  • A single channel (Slack, Teams, whatever) with strict rules about what gets posted there: active incidents only, no general discussion
  • One person declared as incident lead. Not the loudest person. The person who owns the fix
  • A shared doc or Notion page where the current status lives. Updated every 15 minutes during an active incident
  • Clear escalation paths. If the landing page is down, who do we call? If the payment processor is failing, who has the Stripe credentials?

I've run this process for agencies managing 20+ clients. When it works, we go from "something might be broken" to "confirmed, diagnosed, fixed" in under 30 minutes. When it doesn't work, we get the 11-hour checkout outage I mentioned.

The Two Biggest Mistakes Teams Make
